Hartford HealthCare Cancer Institute | American Cancer Society Patient Navigator Program

We believe that having access to information is a vital part of the decision-making process in health and well-being.

Hartford Hospital is privileged to be the first Hospital in New England to partner with the American Cancer Society Patient Navigator Program.

The ACS Patient Navigator helps identify the barriers patients may have in accessing treatment and works in partnership with the cancer clinical team to connect patients to programs and services available at the hospital, through the ACS and in the community. Some of the ways that we can help...

  • Assist in locating local and national resources
  • Coordinate transportation to and from cancer treatment
  • Provide access to wigs, hats and prosthesis
  • Provide cancer information
  • Provide information on ACS, hospital and community programs
