Nurse Navigators

Learning you have cancer can be frightening and overwhelming.

Your doctors, nurses and other care providers are there to help, but it can be hard to process all the information you’re receiving and make choices about your care and treatment.

Nurse Navigators are the welcoming face of the cancer program at Hartford Hospital. Registered nurses with cancer expertise, the navigators are skilled professionals who meet with patients from the moment they come to the hospital for care, and remain by their side throughout their course of treatment and rehabilitation, explaining their care options and answering any questions they may have.

Hartford Hospital’s nurse navigators are here for you. They can help you understand your diagnosis and treatment, and provide moral support when you need it most.

Our Nurse Navigators can:

  • Serve as a liaison between you and your medical team to help you better understand your treatment options
  • Guide you through the health care system
  • Help you find answers to your questions about medical issues, financial and insurance needs
  • Direct you to hospital, community, and American Cancer Society support programs
  • Offer tools to help you keep track of your appointments, lab results, medications, insurance claims and other journey-related information
  • Coordinate transportation to and from cancer treatment
  • Provide emotional support to you and your family by phone and/or face-to-face
  • Arrange interpretation services when there is a language barrier
  • Ensure your access to health services and information
  • Improve your over-all health care experience
  • Reduce the number of delayed or missed appointments
  • Advocate for and ensure timely delivery of early detection and treatment of cancer

Your nurse navigator is available, one-on-one, to help answer your questions, explain your options, or just listen.

Your doctor may refer you to a nurse navigator, or you can call our CANCER CONNECT line at  860.972.6000, Monday - Friday, 8am - 4:30pm.
